Using the Medical form on your School app is a great feature for staff members to have access when they have an activity outside school and parents/guardians have submitted a form to consent to it via the app. This way users will be able to make Medical forms available to teachers for those students who will be joining the activity.
The parent updates the medical form on their School app under the 'My student tab'. Staff members can view medical forms on their Administrators app under a consent form which has been selected to be shared or in the main Dashboard. If you are not seeing the tab on your main Dashboard means you do not have access to it and requires to be added by a full access member. Please contact the School admin or refer to the help guide on Access and permissions.
To view who has submitted/not submitted their medical form please follow the steps below:
- Go to your Dashboard and tap on Reports then in the dropdown select Medical Form list
- Select filters
- Hit search
- To view the information submitted click on the ‘eye icon’
To export medical information please follow the steps below:
- Go to your Dashboard and tap on Reports then in the dropdown select Medical Form list
- Select year and or class
- Hit search
- Tap on Export
Please note that you cannot export the medical information for the whole school, you can only export when applying a filter to a year or a class.
To contact parents/guardians who have not yet submitted the medical form for their student you can easily create a group to send a personal notification requesting them to do so.
- Go to your Dashboard and tap on Reports then in the dropdown select Medical Form list
- Select filter as ‘Not Submitted’
- Select filter as ‘ No medical condition’
- Hit search
- Tap on Create Group
Then when sending your Personal notifications you can select Groups + name of the group you created.
Acknowledge button
It works as a flag for you internally to know another staff member has already seen/dealt with this form.
Ambulance icon
When there is an ambulance icon it is to flag you that the parent/guardian has submitted the form indicating at least one question as Yes for medical condition. These forms should be reviewed by the school so you are aware of the medical condition of that particular student.
No ambulance icon means the parent has submitted the form but there is nothing to be concerned about, you will still have access to medical details for the student.
