The Forms section of the Administrator's App allows assigned staff members to view form submissions, including students' medical information and other details. This feature is particularly useful for tracking form submissions before a trip, managing students during the trip, issuing policy forms, and more.
1. Open your Administrator's App
2. Go to the Main Side Menu
- From an Android Device, tap on the 3 horizontal lines on the top left of the screen
- From an Apple Device, tap on the more tab on the bottom right of the screen
3. Tap on Consent Forms

Here you will see forms that have been assigned upon form creation
Staff can be added to be able to view the form in the Administrator's App from the CMS/Web Version of the system
4. Tap into the Form you wish to view
5. Monitor submissions, medical information and other details. Explained more below.
Consent Forms Main Screen
- View all of your assigned forms via the screen below after tapping on Consent Forms
- You will see who Forms received, forms not received (Due) and those with consent given (Permitted)
Form Detail
- Filter by Received, Not Received and All
- Preview the Form content to see what it includes
- See the last date of submission
- View all students assigned to the form along with their year, class and medical information status (first image below)

The Medical Information Status icon on the right of the student can be either:
- Green - Submitted Medical Form with no medical condition
- Red - Submitted Medical Form with a medical condition
- Grey - Medical Form not yet submitted
- The Red (Medical Condition Submitted) Status icon can be tapped and from that, you will see exactly what condition this student has
- This makes it very easy to quickly find out the condition if something was to come up during a school trip or even on school grounds
Form Detail Continued
- When tapping on the Not Received tab, we can see who has not submitted the form
- We can also see whether or not the student has parental representation on the app
- The red rectangle to the right of the student means parent/guardian is not an app user
- The green rectangle to the right of the student means the parent guardian is an app user

If we take the below image, for the first student (Adam Whitney):
- the primary parent/guardian is not an app user
- the secondary parent/guardian is an app user

- Tap into a student name who has submitted the form
- View the completed form with responses from the parent/guardian for this particular student
