Reminders

Choose a frequency that suits the deadline. Reminders go only to users who have not completed the form and continue until the last submission date.

SMS reminders incur charges. Check the reminder frequency and SMS option before submitting.

Last Date of Form Submission

Users cannot submit after this date. You can edit the date later if you need to extend the deadline.

One Per Family

Assigns the form to one student per family, using the parent’s mobile number to identify the family. Select this when a parent only needs to respond once for all their children.

Adds an overall consent question, allowing the user to choose whether they give permission. This makes consent responses easy for staff to review.
