How to duplicate an existing Form

Forms can be used for a variety of needs as you can create questions based on each individual case.

You will be required to create the forms under your main Dashboard, but responses can be managed from either the Dashboard or the Administrator's app.

 

To duplicate an existing form please follow the steps below:

  1. Go to your Dashboard and tap on Forms then in the dropdown select Manage Forms
  2. Find the form you wish to duplicate and tap on the duplicate icon (far right column)
  3. Select who needs to complete the form i.e. Parent/Guardian
  4. Make any changes to the Title or Form content if needed
  5. Select Reminders
  6. Add new questions or make any changes to questions if needed
  7. Enter the last date of submission
  8. Enter the name of the teacher who will manage the form so they can view on their Administrators app
  9. Select One Per Family (if applicable)
  10. Filter by the dropdown menu
    • Groups: If you have created a group previously you can find by the name of the group
    • Students: Will show list of year/class where you can select the ones relevant to you
    • Family Type: You can select year/class and then apply family type filter
  11. Apply filter
  12. Select all or select relevant students that you wish to submit the form to
  13. Add selected so they move to your Final list
  14. Select the final settings on your form (if applicable)
  15. Preview: You can view how the form will work for users and test answers to ensure you are happy before submitting
  16. Submit

 

 

Reminders  
Please note that the setting will change depending on how soon you need the response from users (i.e. I am sending consent for a Trip the following week I might select reminders to go every 2 days). Reminders will only notify users who have not yet filled out the form, users will then receive reminders until the last submission date.

Last date of Submission  
This means users will not be able to submit the form after the date selected here. You can update the last date of Submission after creating the form if you want to allow parents a few extra days

One per family  
This setting will assign the form to only one student per family, based on the mobile number of the parent. So if you only require the parent to sign a form once for all their students, please do tick the box.

Require consent  
This box will display a default question to the user if they do or do not consent to the form they are signing. It is a great tool if all you need from the user is to consent as it will display quite easily the responses for staff members on their Administrators app.

Show Medical Details  
Teachers who are assigned to view the form on their Communicators app will be able to see medical forms on their Communicators app if this box is selected. Please note teacher will only view it if the Medical form has been submitted by the parent.  
On your Administrators app, you will see a cross beside the student's name with different colours which refer to:  

  • Red - does have a medical condition you need to be aware  
  • Green - The parent has submitted but it does not have a medical condition you need to be aware of  
  • Grey - The parent has not yet submitted medical information

 

 

Send SMS  
This feature will identify users who don't have the school app and only those will receive an SMS with the message and links to download the school app. Everyone else will receive a notification via the School app.

Send notification  
Always tick Send notification as you want users to know there's a form that they need to sign. This will send a 'beep' onto the user's phone bringing them into the form to fill.

Email notification  
By ticking this box teachers who are assigned to view the form on their Administrators app will receive an individual email every time a form has been submitted.

Optout  
If you have an optional form you can give parents the option to Optout. This way parents will not be getting reminders of the form if they do not wish to submit it. (i.e.Trip to City Centre) Do not give the Optout option if the form is mandatory (i.e. Policies)
